How reliable are the classic clinical signs of bacterial meningitis?

According to international sources, the classic clinical signs and provocative bedside maneuvers have very poor diagnostic sensitivity when considered individually or as a complete triad, meaning their absence never safely excludes acute bacterial meningitis. The classic triad of fever, neck stiffness (nuchal rigidity), and altered mental status is present in only 23% to 51% of adult episodes. Classic bedside maneuvers demonstrate remarkably low sensitivity—ranging from 5% to 11% for Kernig's sign, 5% to 9% for Brudzinski's sign, and roughly 30% to 63% for isolated neck stiffness.

In international guidelines, Kernig's and Brudzinski's signs are considered unreliable for clinical diagnosis. However, diagnostic reliability improves substantially when evaluating broader combinations of features: 85% to 96% of adults with acute bacterial meningitis present with at least two of the four cardinal features (fever, headache, neck stiffness, and altered mental status). The complete absence of all four features carries a negative predictive value near 100%, whereas presenting with one or none carries a negative predictive value of 98.9%. Classic signs are also significantly less pronounced in older adults, neonates, and immunocompromised hosts, who frequently present atypically with isolated confusion, delirium, or systemic sepsis without prominent meningism.
